Transcript Request Form

Two types of transcripts may be requested.

The first is a student copy which will be clearly marked as such. This type of transcript may only be sent to or picked up by the student or someone designated by the student. This is NOT an official transcript.

The second is an official transcript. The official transcript may ONLY be sent directly to the institution desiring the information. The student is NOT ALLOWED to be a carrier of the official transcript.

We charge a $5.00 processing fee for student or official transcripts.

Graduates receive one free transcript after graduation. This free transcript may be requested as a student copy or an official transcript.

Details to make a payment for your transcript will be provided to you via email after we review your request.

Ladies please provide your maiden name if you have married since attending Landmark Baptist College.

I would like an official transcript emailed to an institution and the institution allows email delivery of transcripts.

Payment

This is my Free copy. (The first copy for graduates of Landmark Baptist College is free.)
I will pay $5 for my transcript. (We will send you an invoice through email to pay.)
